1. Participation

I understand that this amount is preferably due two weeks from today.

2. To ensure timely participation placement:
Please sign this agreement and return by October 1st to be a sponsor of the annual walk/event which is in November. To be included in the January newsletter, the deadline to return sponsorship forms is December 1st.

prominently listed in our printed materials.
